Basic Function:            

The Supervisor provides direction and leadership to nursing staff to ensure the highest quality of care for all residents.    


Responsible To:            Assistant Director of Nursing, Director of Nursing

Education, Qualifications, and Credentials:

  • Must possess a current Indiana license as a Licensed Practical Nurse or as a Registered Nurse and have graduated from an accredited school of nursing. Two years of professional nursing experience in the long term setting is required. Must possess and demonstrate a high degree of leadership, organization ability, and communication skills. Current CPR certification is preferred.
  • All applicants will undergo a drug screen, criminal history check, and history and physical as part of the interview process.

Duties and Job Functions:

The employee must be able to perform each essential function effectively to be successful in this position.

  1. The supervisor will determine staffing needs and resolve issues as they arise such as taking sick calls and making sure that each shift is adequately staffed.
  2. The supervisor will approve of switching of work assignments and down staffing.
  3. All incident reports will be directly reported to the supervisor, including work related injuries and incidents.
  4. All changes in condition will be reported to the supervisor.
  5. The supervisor will hold staff accountable for following Heritage of Fort Wayne’s Policies and Procedures and Employee Handbook.
  6. The supervisor will give facility tours to visitors and families if requested.
  7. If needed, the supervisor will round with the physician and nurse practitioner.
  8. Make routine rounds to all areas of the facilities and all the units.
  9. Assist with admits and discharges.
  10. Notify Admissions Coordinator regarding any possible new admits.
  11. Check mailbox in Crofton work room for any pertinent information or communications from management.
  12. Complete any other duties as assigned by management. (i.e. Review 60 day rewrites, audits, enter orders as needed, etc.)
